If your loved one has given the gift of life to others, our Aftercare team may be able to provide you with further recipient information.

All families will receive an outcome letter approximately six weeks after donation. If you are seeking more information following that letter, please complete the form below. One of our Aftercare Specialists will contact you just as soon as we can.

**This type of information can only be released to the donor’s legal next-of-kin. If further permission is required before our staff can obtain an update for you, we will contact the donor’s legal next-of-kin directly. If you have any questions about this process, please contact AC@neds.org **
